What were they doing?

Did they get treatment?

What the reporter understood at the time. It is context for the recordability check, not an answer to it — whether this was treatment beyond first aid under 1904.7(b)(5) is decided in the wizard.

Who is reporting this?

Filing this is not a decision about whether it goes on an OSHA log. It is a record that it happened, timestamped, sent to the person who reviews them.
