The moment a report reaches the employer is generally the “employer knowledge” event that starts the OSHA seven-calendar-day window to record a case (29 CFR 1904.29(b)(3)) and, in California, the one-working-day obligation to give the employee a DWC-1 (Labor Code 5401(a)). This stamp is what makes the intake → triage → case → DWC-1 chain a defensible timeline.
Informational only. Whether a given report constitutes employer knowledge, and what follows from it, is the employer's determination — EHM assists, it does not decide.
Triage — what the employer decided this report is
Log as a near-miss
Nobody was hurt. Kept as a leading indicator — the near-misses are what predict the injury that has not happened yet.
“Strap failure. Rack re-strapped and toolbox talk given the same morning.”

A decision is kept, not replaced. If the picture has changed, record a correction on the case rather than re-deciding the report.
