Cal/OSHA Form 300A
Client Admin ▸ Safety ▸ Fresno Distribution Center ▸ 2026
Column names and order follow OSHA's published 300A CSV template. OSHA revises it between filing seasons — check the current template on the ITA site before submitting.
Cases for 2026 have been corrected since Dale Whitfield signed it on 2026-09-16. The figures below are the ones that were signed and have not been altered — a certified summary is frozen, which is why this has to be said rather than quietly corrected:
- Other recordable cases (J): signed as 0, the log now says 1
- Injuries (M1): signed as 1, the log now says 2
Do not post or submit this version. Superseding a certified summary needs a versioned 300A, which is not built yet — until then this is flagged rather than worked around.
Total recordable cases: 1 · counted from recordable cases whose determination the employer has confirmed.
“I certify that I have examined this document and that to the best of my knowledge the entries are true, accurate, and complete.”
A company executive must certify this summary. Under 29 CFR 1904.32(b)(4) that means an owner, an officer of the corporation, the highest-ranking company official working at the establishment, or that person's supervisor.
Posting. Post this summary in a visible place where employees can see it, from February 1 to April 30 of the year following the year covered.
